Hope Renewed Youth Conference 2016

Event Time : June 10, 2016, KICK-OFF 1PM
Thank you for your interest in our 12th annual conference, geared toward ages 11-18 years of age. This is a 3-day event, kicking off on Friday, June 10, 2016- 1:00 PM at Riverplex Recreation. It will conclude on Sunday, June 12, 2016 at Illinois Central College Performing Arts Center.
We will be providing (2) $500 State Farm Hope Scholarships, recipients are selected based on a written essay, attendance to all the conference sessions and proof of acceptance at a 2 or 4 year accredited college or university. (View past winners Photo Gallery)

PARENTAL PERMISSION
The registration form must be completed and signed to participate in Hope Renewed Youth Conference. Registration deadline for 2016 is May 3, 2016.
(Please mail completed form 3-5 working days before the deadline. As parent(s)/guardian(s), We fully understand the nature of Hope Renewed Youth Conference. We give permission for my child to participate.
Total cost of the event is $10 per youth (if registered by May 3, 2016) $15 after that date.

I also release all photography to be used by Hope Renewed Conference, its sponsors and affiliates.
This permission slip releases the Hope Renewed Youth Conference Committee, Peoria Park District, ICC
Performing Arts Center, their employees and volunteers from
any claims by or on behalf of participants should injury or loss
